POLMED (South African Police Service Medical Scheme)
Restricted scheme · Established 1967 · For active and retired SAPS employees · Administered by Medscheme
Overview
POLMED is a registered non-profit restricted medical scheme regulated by the Council for Medical Schemes under the Medical Schemes Act 131 of 1998. Established in 1967, POLMED exclusively serves active and retired employees of the South African Police Service (SAPS) and their qualifying dependants. With nearly six decades of continuous operation, it is one of South Africa's longest-established restricted schemes.
The Scheme's governance structure reflects the SAPS institutional hierarchy: the Board of Trustees comprises seven members elected by SAPS employee-members and seven appointed by the National Commissioner of Police, ensuring balanced representation of employee and institutional interests. Medscheme (an AfroCentric Group subsidiary) administers the Scheme on behalf of the Board, providing managed-care infrastructure, hospital tariff negotiations, and managed disease programmes across all plan options.

Membership Profile
POLMED serves active SAPS employees and their dependants, along with retirees who continue their membership into retirement. The Scheme is one of the larger restricted schemes in South Africa by beneficiary count. SAPS employment is broadly distributed across all provinces and covers a wide income range, from entry-level constables through to senior general officers — a span reflected in the Scheme's multi-tier plan structure.
The membership age profile is shaped by SAPS demographics: a significant proportion of younger, active members (active police officers) combined with a retiree base that has grown as the Scheme has matured over nearly 60 years. Detailed principal-member and beneficiary counts are published in POLMED's annual report, available from polmed.co.za.

Financial Health
POLMED reports a solvency ratio above the 25% statutory minimum required under Regulation 29(2) of the Medical Schemes Act. The Scheme faces the same structural cost pressures common to all South African medical schemes — medical inflation exceeding CPI, increased utilisation of high-cost specialist care, and chronic disease burden — while navigating the funding dynamics of a restricted SAPS member pool.
A well-structured investment policy supports the Scheme's reserves, providing a buffer that moderates the impact of adverse claims years on contribution increases. The SAPS as employer also contributes toward eligible employees' POLMED contributions, which reduces the net member cost and supports sustainable membership levels.
POLMED's detailed financial statements — including solvency ratio, accumulated funds, net claims ratio, and annual contribution-increase rationale — are published in the annual report at polmed.co.za.

Plans & Options
POLMED offers a range of benefit options designed to align with SAPS salary bands and family configurations, from a focused hospital plan for lower-income members through to a comprehensive option for senior officers. Exact plan names and contribution rates for 2026 are published in the POLMED 2026 Member Guide at polmed.co.za. The Scheme's contribution structure reflects both the member's own salary band and a concurrent SAPS employer subsidy.

Key Benefits & Differentiators
SAPS-specific scheme design. POLMED's benefit structures, contribution schedules, and eligibility rules are purpose-built around SAPS conditions of service — including the income range, shift patterns, geographic distribution across all 9 provinces, and occupational risk profile of police employees. This tailoring produces a more appropriate fit than a generic open-scheme product for this membership.
SAPS employer subsidy. SAPS as the employing authority contributes a concurrent portion toward eligible employees' POLMED contributions. This employer contribution materially reduces the out-of-pocket cost of membership for active members, making comprehensive options accessible at net costs below equivalent open-scheme premiums.
PMB compliance and chronic disease management. All POLMED plans cover the 27 Prescribed Minimum Benefit Chronic Disease List conditions at legislated cost. The Medscheme administration platform provides structured chronic registration and disease management programmes for high-prevalence conditions including hypertension, diabetes, asthma, and HIV/AIDS.
Medscheme managed-care infrastructure. Through Medscheme, POLMED members access one of South Africa's largest hospital tariff networks, electronic pre-authorisation, pharmacist-managed formularies, and disease management case workers. This infrastructure supports efficient claims processing and proactive case management for complex conditions.
Retiree-accessible membership. Unlike employer group health cover in the private sector (which typically terminates on retirement), POLMED allows qualifying retirees to continue their membership into retirement — a critical feature for a service-sector workforce with an established retirement benefit culture.

Wellness & Loyalty
POLMED includes preventive-care and wellness benefits on all plans, administered through Medscheme's wellness platform. Members have access to annual health screenings, HIV counselling and testing, and chronic condition management support as part of the standard benefit package. The scope of wellness benefits varies by plan option — higher-tier plans include more extensive screening protocols.
POLMED does not operate a standalone consumer wellness rewards programme (such as Vitality). The wellness offering is structured around occupational health and disease management priorities appropriate to a large, geographically distributed public-safety workforce.

Claims & Service
Claims are administered by Medscheme via electronic provider submissions, the POLMED member portal, the Medscheme app, and manual submission. Medscheme's national call centre handles member queries, pre-authorisation requests, and claims enquiries.
Pre-authorisation for elective procedures and specialist referrals is handled via Medscheme's clinical review team. Dispute resolution follows the Medical Schemes Act process: internal dispute committee → CMS adjudication for unresolved matters.

How the Scheme Is Set Up
POLMED is available only to active and retired SAPS employees and their qualifying dependants. Within that eligible population:
Junior officers and constables (entry-level plan): The entry hospital plan with employer subsidy provides meaningful PMB-compliant cover at a net member cost that reflects a constable's salary band. The SAPS employer subsidy is particularly valuable at lower salary levels, where the scheme cost would otherwise represent a disproportionate share of take-home pay.
Mid-career officers with families (mid-tier and comprehensive options): SAPS members with families who need broader day-to-day and chronic cover are well served by the mid-tier options. Structured chronic management, GP access, and in-hospital cover combine effectively for a police officer household in this life stage.
Senior officers and officials (senior option): Senior SAPS members requiring top-tier cover — broader specialist access, higher annual limits, any-provider flexibility — can access the scheme's top-tier option at net costs that reflect their salary-banded contribution and employer subsidy.
SAPS retirees: Continuation membership is a critical feature for retiring SAPS employees who would otherwise need to transition to an open scheme with no employer subsidy at a financially vulnerable moment. Qualifying retirees should confirm continuation conditions directly with POLMED before retirement.

Eligibility & Continuation
Who can join: Employees appointed under the South African Police Service Act (Act 68 of 1995) and their qualifying dependants. This includes all active SAPS uniformed members — from constables through to the National Commissioner — and civilian employees of SAPS (administrative, technical, and support staff).
Employer subsidy: SAPS as employer contributes a concurrent portion of eligible members' contributions. The amount varies by plan option and salary level; members should confirm the applicable employer portion with SAPS Human Resources before selecting a plan option.
On leaving SAPS: Active members who leave SAPS other than through qualifying retirement lose eligibility and must transfer to an open medical scheme within the Medical Schemes Act's portability period. Portability provisions protect existing conditions — a new scheme cannot apply waiting periods for conditions already covered under POLMED.
Retirees: Members who retire from SAPS service (on pension, medical retirement, or after qualifying service period) are typically eligible to continue POLMED membership as continuation members. Specific conditions are set out in the POLMED Scheme Rules; retirees should apply for continuation status before their SAPS employment terminates.
Dependants: Qualifying dependants (spouse/life partner, dependent children as defined in the Scheme Rules) remain on POLMED while the principal member is enrolled. On the principal member's death, continuation rules govern the surviving spouse's and children's ongoing cover.
Governance: The Board of Trustees comprises seven members elected by SAPS employees and seven appointed by the National Commissioner of Police. This governance model is defined in the POLMED Scheme Rules and ensures both employee-elected and institutional representation at Board level.
