About Depression
Major depressive disorder (MDD) is characterised by persistent depressed mood, loss of interest or pleasure, and a range of cognitive and physical symptoms — fatigue, sleep disturbance, concentration difficulty, appetite change, and psychomotor changes — lasting at least two weeks and causing significant functional impairment.
Depression is the most prevalent condition in the AURA Phase 1a condition set by absolute number of South Africans affected. A national survey found 25.7% probable depression — more than one in four South Africans. One in seven Discovery Health Medical Scheme members actively claimed for a mental health condition in 2024, a 46% increase over 12 years.
Depression is highly treatable. Most people with moderate depression respond to a combination of antidepressant medication and psychotherapy (CBT) within 3–6 months.
How Depression Is Classified
Depression is assessed using validated scales including the PHQ-9 (Patient Health Questionnaire, 9 questions):
| PHQ-9 Score | Severity |
|---|---|
| 5–9 | Mild |
| 10–14 | Moderate |
| 15–19 | Moderately severe |
| 20–27 | Severe |
Treatment Options
First Line — SSRIs (Selective Serotonin Reuptake Inhibitors)
SSRIs are first-line pharmacological treatment for major depression. All are available as inexpensive generics in South Africa.
- Escitalopram 10–20mg daily — ~R90/month (generic)
- Sertraline 50–200mg daily — ~R100/month (generic)
- Fluoxetine 20–60mg daily — ~R80/month (generic)
Full antidepressant effect takes 4–6 weeks. Response is assessed at 4–8 weeks. If inadequate response at adequate dose: switch to another SSRI, or step up to SNRI, or refer to psychiatrist.
Second Line — SNRIs and Other Antidepressants
- Venlafaxine XR 75–225mg daily — ~R200/month (generic). Also effective for anxiety, chronic pain.
- Duloxetine 30–120mg daily — ~R250/month (generic). Dual action; useful with comorbid pain.
- Mirtazapine 15–45mg nightly — ~R150/month. Sedating; improves sleep and appetite.
Psychotherapy — CBT
Cognitive behavioural therapy is as effective as antidepressants for mild-moderate depression and superior for relapse prevention. Sessions: typically weekly for 8–20 weeks. Private psychologist rate: R1,000–R1,500 per session in SA.
Specialist Management (Severe/Treatment-Resistant)
- Psychiatrist assessment
- Antidepressant augmentation (lithium, quetiapine, aripiprazole)
- Electroconvulsive therapy (ECT) for severe, refractory, or psychotic depression — inpatient procedure; safe and highly effective but carries stigma
Annual Cost Estimates (Private Sector)
| Severity | Profile | Estimated Annual Cost |
|---|---|---|
| Mild | SSRI + 12 CBT sessions + 2 GP visits | ~R14,080 |
| Moderate | SNRI + 20 CBT sessions + 4 psychiatrist visits | ~R29,000 |
| Severe / TRD | ECT course + augmentation + inpatient admission | R45,000–R81,600 |
The Critical Coverage Distinction: Depression Is NOT CDL
This is the most important advisory point for depression:
Depression is a PMB condition (code 902T — major affective disorders), NOT a CDL condition. This distinction changes everything:
What PMB covers (902T — the two are alternatives, not additive): - Up to 21 in-hospital days per year (acute psychiatric admission, inpatient ECT), OR - Up to 15 outpatient psychotherapy sessions per year in lieu of admission
What PMB does NOT cover: - Antidepressant medication (SSRIs, SNRIs) prescribed after hospital discharge - Outpatient psychotherapy sessions beyond 15 per year - GP consultations for depression management
Antidepressants are funded from your scheme's day-to-day (d2d) benefit — not from CDL chronic medication benefit. Most schemes fund generic SSRIs as routine chronic medication through d2d or a separate mental health programme. But coverage is not guaranteed and varies by plan.
Scheme Mental Health Programmes
Beyond basic PMB, several schemes offer enhanced mental health benefits: - Discovery Health: Mental Health Care Programme — structured programme with additional sessions and case management - Bonitas: Dedicated depression/anxiety/PTSD/alcohol abuse programme - Medihelp: Depression programme and out-of-hospital psychologist visits
For patients with ongoing depression, the scheme's mental health programme quality is a meaningful advisory variable.
Gap Cover Relevance
| Scenario | Gap Cover Relevance |
|---|---|
| SSRI/SNRI antidepressant costs | NONE — d2d funded, not a gap issue |
| Outpatient psychotherapy sessions | NONE — d2d or PMB funded |
| Inpatient psychiatric admission: psychiatrist shortfall | MEDIUM — psychiatrist billing above scheme rate |
| ECT inpatient: facility + anaesthesiologist shortfall | MEDIUM |
Depression is not a high gap cover condition. The primary advisory question is scheme mental health benefit quality — specifically, how many psychotherapy sessions are funded and whether the d2d benefit covers long-term antidepressant use.
SADAG: National Support
The South African Depression and Anxiety Group (SADAG) operates over 30 national helplines. Crisis line: 0800 456 789 (24 hours). General helpline: 011 234 4837 (daily 08:00–20:00).
What the PMB covers, in the regulation’s terms
- For major depression and bipolar disorder, the PMB is up to 3 weeks (21 days) a year of hospital-based care, including electroconvulsive therapy (ECT), or up to 15 outpatient therapy sessions instead of an admission. It is one or the other, never both. (902T. Medical Schemes Act regulations, Annexure A; outpatient alternative from the CMS PMB guideline (v1, 30.09.2020).)
