Herniated Disc or Facing Spinal Surgery? Here's What to Do About Your Medical Aid

Whether you've been diagnosed with a disc herniation and are starting conservative management, or you've been told surgery is necessary, medical aid navigation matters enormously. Spinal surgery is one of the most expensive procedures in private medicine — and the gap cover shortfall can easily exceed R100,000.

The Honest Starting Point

Spinal surgery is PMB-covered when surgical criteria are met. The challenge is not getting the surgery funded — it's managing the gap between what your scheme pays and what your neurosurgeon or spinal surgeon charges. That gap is large.

If you are in conservative management: your plan is to document treatment properly, exhaust conservative options, and build the clinical record that will support a PMB surgical pre-authorisation if needed.

If surgery has been recommended: your priority is gap cover notification, surgeon shortfall awareness, and correct PMB coding.

Step 1: Get an MRI — Confirm the Diagnosis

Conservative management for non-specific back pain can be funded from day-to-day. But surgical planning requires an MRI.

To get your MRI funded: - Ask your GP for a neurosurgeon or spinal orthopaedic surgeon referral before the MRI where possible — a specialist referral strengthens the pre-auth. - Request MRI pre-authorisation from your scheme: "I have a suspected lumbar/cervical disc herniation with radiculopathy. My GP is referring me to a neurosurgeon. Please confirm MRI [lumbar/cervical] spine pre-authorisation." - MRI spine cost: R6,000–R12,000. Usually covered under radiology benefit; if a surgical indication is established, it is a PMB diagnostic.

Step 2: Start a Documented Conservative Management Programme

For non-emergency disc herniation, your scheme will require documented failure of conservative management before authorising elective surgery. This is not just a bureaucratic hurdle — it's clinically appropriate: most disc herniations resolve with 6–12 weeks of proper management.

Document everything: - Physiotherapy: Request monthly progress notes from your physiotherapist — documenting initial presentation, ongoing symptoms, and response (or lack thereof) to treatment. - NSAID course: Confirm your GP has documented the duration of NSAID therapy and outcome. - If an epidural steroid injection was performed: document the outcome (temporary/partial/no benefit).

The record you are building is the clinical motivation your neurosurgeon will use for the surgical pre-authorisation.

Step 3: Obtain a Neurosurgical Opinion and Formal Motivation

Your neurosurgeon's letter is the cornerstone of the surgical pre-auth. It must contain: - Clinical diagnosis (e.g., "L4/5 right-sided disc herniation with L5 radiculopathy") - ICD-10 code(s): M51.1 (lumbar/other intervertebral disc derangement), M50.1 (cervical disc derangement with radiculopathy), M47.0 (spinal stenosis) - MRI findings (attached report) - Conservative treatment history and failure - Recommended surgical procedure with CPT/procedure codes - Clinical necessity statement

Ask the neurosurgeon's rooms to send this directly to your scheme's medical director. Do not rely on the call centre to assess complex spinal indications.

Step 4: Get a Quotation and Confirm Gap Cover Coverage

Before any spinal surgery booking:

  1. Quotation from the neurosurgeon: Request an itemised quotation showing procedure codes and rand amount per code. Ask what percentage of scheme tariff they charge (e.g., "200% of scheme rate").
  2. Quotation from the anaesthesiologist: Get the anaesthesiologist's rate separately.
  3. Hospital cost: Most spinal surgery hospital costs are scheme-funded directly (PMB). The surgeon and anaesthesiologist shortfall is where gap cover applies.
  4. Contact your gap insurer: "I am having [procedure] at [hospital] with Dr [name]. The surgeon quotes [X%] of scheme rate. Please confirm my OPL for this procedure and pre-notify the claim."

Check your gap cover OPL limit. For complex fusion surgery, confirm your OPL is sufficient. Standard policies: R180,000–R220,000/year. If the total shortfall exceeds your OPL, you fund the balance OOP.

Step 5: Choose a DSP Hospital

Your scheme has Designated Service Providers (DSP hospitals). Confirm the surgery will be performed at a DSP.

Non-DSP hospital admission: you may face a co-payment of 20–30% of total hospital cost. For a multi-day spinal fusion admission, this can be significant.

Most large private hospital groups (Netcare, MediClinic, Life Healthcare) are DSPs for major schemes. Confirm with your scheme.

Step 6: Understand the Post-Surgical Rehabilitation Reality

Be realistic about post-op physio funding:

After spinal surgery, your scheme typically funds 5–10 physiotherapy sessions under the acute surgical rehabilitation benefit. Recovery from complex spinal fusion requires significantly more — 20–40 sessions over 3–6 months.

Plan for OOP physiotherapy costs: R600–R1,200/session. Budget for at least 15–30 sessions beyond your funded allocation.

Day-to-day ATB: After exhausting your MSA, your ATB kicks in for additional day-to-day services. Confirm your plan's ATB threshold and accumulation rules.

If Your Pre-Auth Is Declined

  1. Request written reasons — ask for the specific clinical and benefit reasons.
  2. Have your neurosurgeon contact the scheme's medical director directly to discuss the clinical motivation.
  3. File a formal appeal citing the PMB DTP for spinal surgery with documented conservative management failure.
  4. If progressive neurological deficit (worsening weakness/foot drop): escalate urgently — clinical deterioration is grounds for expedited review.
  5. Council for Medical Schemes: 0861 123 267 — for PMB disputes.

Key Calls to Make

Call What to say
Scheme "I need pre-authorisation for [procedure + ICD-10] for a documented disc herniation with failed conservative management. Can you confirm this as a PMB DTP and approve the surgery at [DSP hospital]?"
Gap cover "I am having [procedure] at [hospital] on [date] with Dr [name] charging [X%] of scheme rate. Please pre-notify and confirm my OPL balance."
Hospital "Is [hospital] a DSP for [scheme name]? Can you confirm co-payment status for spinal surgery admissions?"
