Thyroid Cancer — Urgent Situations and What Your Medical Aid Covers

If you have neck swelling causing difficulty breathing or swallowing, sudden neck pain after thyroid surgery, or signs of hypocalcaemia — go to the nearest emergency room immediately. Call 10177 for breathing difficulty.

Thyroid cancer itself is rarely an acute emergency. However, thyroid surgery and RAI treatment carry specific complications that require urgent recognition.

Post-Thyroidectomy Emergencies

Hypocalcaemia (Post-Thyroidectomy)

The parathyroid glands lie adjacent to the thyroid and control calcium homeostasis. Total thyroidectomy can inadvertently damage or remove the parathyroid glands, causing acute hypocalcaemia (low calcium) — sometimes within hours of surgery.

Symptoms of hypocalcaemia: - Tingling or numbness around the mouth, fingertips, toes - Muscle cramps, spasms, or twitching (carpopedal spasm — fingers curl, wrist flexes) - Chvostek's sign (tapping in front of the ear causes facial muscle twitching) — nurses check for this post-operatively - Trousseau's sign (BP cuff inflated causes hand to cramp) - In severe hypocalcaemia: laryngeal spasm (difficulty breathing), seizures

Action: Go to the ER immediately or call the nurse/ward immediately if you are still in hospital. IV calcium gluconate is the emergency treatment.

After discharge: If you have tingling around the mouth or muscle cramps after going home from thyroidectomy — go to the nearest ER, not the GP. This is a genuine medical emergency.

Post-surgical hypocalcaemia management (calcium + calcitriol) is funded as a PMB post-operative complication.

Post-Operative Haematoma

Rarely, bleeding into the thyroidectomy wound causes a haematoma that compresses the trachea (windpipe).

Symptoms: Rapid neck swelling after thyroid surgery, increasing difficulty breathing, stridor (high-pitched breathing noise).

Action: Call 10177 immediately. Post-thyroidectomy haematoma is a surgical emergency requiring immediate return to theatre for wound evacuation. Do not wait.

This is PMB-covered as a post-operative surgical complication.

Recurrent Laryngeal Nerve Injury

Thyroidectomy carries a small risk of recurrent laryngeal nerve injury, causing hoarseness or voice change. Bilateral nerve injury (rare with total thyroidectomy) can cause airway compromise.

Action: Persistent hoarseness post-thyroidectomy is expected and usually temporary. If you develop progressive breathing difficulty or inability to phonate/swallow: ER immediately.

Anaplastic Thyroid Cancer — An Emergency in Itself

Anaplastic thyroid carcinoma (ATC) is rare (<1% of thyroid cancers) but the most aggressive — it can grow rapidly over days to weeks, causing airway compression.

Symptoms: Rapidly enlarging, hard, fixed neck mass; difficulty swallowing; shortness of breath; hoarseness.

Action: Urgent oncology assessment — same week. ATC is an oncological emergency; rapid multi-modality treatment (surgery, radiotherapy, systemic therapy) offers the best chance. This is PMB-covered under the cancer DTP.

Thyroid Storm — Rare But Life-Threatening

Thyroid storm (thyrotoxic crisis) is an extreme hyperthyroid state — not related to thyroid cancer directly, but it can occur if thyroid cancer is associated with significant hyperthyroidism or rarely after RAI treatment in large hyperfunctioning tumours.

Symptoms: Very fast heart rate (>140 bpm), high fever (>40°C), agitation/confusion, profuse sweating, vomiting, diarrhoea.

Action: Call 10177 immediately. Thyroid storm has high mortality without urgent ICU treatment (antithyroid drugs, propranolol, hydrocortisone, supportive care).

If Levothyroxine Is Interrupted

Do not stop levothyroxine. Post-total thyroidectomy, you have no thyroid function at all. Levothyroxine interruption will cause progressive hypothyroidism within weeks.

If your CDL levothyroxine prescription is declined or interrupted: 1. Contact your scheme immediately — CDL medication cannot be denied for a registered CDL condition 2. Generic levothyroxine is available at most pharmacies for R80–R200/month OOP if needed as a bridge 3. Council for Medical Schemes: 0861 123 267 — for CDL medication denial

If you are trying to keep your TSH suppressed (post-thyroid cancer), sudden levothyroxine interruption also removes the TSH suppression that reduces recurrence risk. This is a clinical urgency — not just a comfort issue.

If Thyroglobulin Rises (Recurrence Alert)

A rising thyroglobulin (Tg) during surveillance — particularly with negative anti-Tg antibodies — is a marker of possible thyroid cancer recurrence.

This is not an emergency but it is an oncological urgency — within days, not weeks: 1. Contact your oncologist or endocrinologist immediately 2. Arrange diagnostic RAI whole-body scan or FDG-PET (for RAI-refractory disease) 3. MDT review 4. All recurrence investigations and treatment are covered under the PMB cancer DTP
